New Patients

Welcome to ProActive Physical Therapy!

New patients,

Once you have received a prescription for physical therapy from you physician, please call our friendly staff and they will assist in verifying your insurance and scheduling your initial evaluation. Your initial visit will last approximately one hour, as will each following session.

You may assist us in providing optimal care by doing the following:

1. Please download and print the required forms, fill them out and bring them to your initial visit.
2. Arrive 10-15 minutes prior to your scheduled appointment time to ensure the accuracy of the paperwork and obtain any needed insurance information.
3. Wear or bring comfortable clothes that will allow you to move and allow your therapist access to the area being treated.

Please note that out of respect for our other clients, and to maintain the high quality of care we provide, we ask that you provide 24 hour notice of any cancellations. Also understand that if you are late for your appointment your treatment may be reduced in order to not affect the subsequent clients treatment.
