Insurance

The following is a list of insurances currently accepted by ProActive Physical Therapy as in-network. If your insurance is not listed, please call and our staff will be happy to call and verify your benefits to see if we would be able to work with your plan. We also offer a self-pay rate and are willing to establish a payment plan with our clients. Our billing department will file a claim with your insurer for our services provided. For deductibles, co-pays, and any services not covered by your insurance, we accept cash, check, Visa, MasterCard, debit cards, etc. Please note: Payments for deductibles and co-pays are due at the time of service.
